About The Position
At WelbeHealth each participant of our program is guided by our interdisciplinary team (IDT) - composed of clinical and non-clinical members who work cohesively to provide quality patient-centered care. The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety. Requirements
- Bachelor’s degree in physical therapy; master’s or doctoral degree preferred
- Two (2) years in clinical setting with frail or elderly population preferred
- Valid California Physical Therapy License, or proof of license eligibility
- Current CPR certification preferred
Nice To Haves
- We are seeking Physical Therapists that ideally have outpatient, acute, inpatient, or skilled nursing experience.
Responsibilities
- Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
- Actively participate in IDT meetings to develop participant care plans
- Deliver and document interventions as agreed upon in participant care plans
- Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
- Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
- Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment
