About The Position

The Physical Therapist I at Franciscan Senior Health and Wellness PACE is responsible for evaluating and treating patients according to legal and professional guidelines, as well as hospital and departmental policies. This role involves instructing and directing Physical Therapist Assistants and Rehab Technicians. The Physical Therapist evaluates and implements care plans to improve, restore, and enhance physical functional performance and promote injury prevention, including documentation and quality improvement. Requirements

  • Bachelor's Degree or Master's Degree in Physical Therapy
  • Cardiopulmonary Resuscitation (CPR) - American Heart Association
  • Physical Therapist License (PT) - State Licensing Board

  • Doctorate in Physical Therapy
  • 1 Year Physical Therapy Experience

Responsibilities

  • Provide evaluation and treatment of the patient within the parameters of legal and professional guidelines, and hospital and departmental policies and procedures.
  • Instruct and direct Physical Therapist Assistants and Rehab Technicians as assigned.
  • Evaluate and implement the plan of care to improve, restore and enhance physical functional performance, and promote injury prevention.
  • Perform documentation and quality improvement, as outlined in departmental policies and procedures.
  • Perform comprehensive physical therapy evaluations, treatment and related follow-up services including discharge planning.
  • Develop a treatment plan with measurable goals based on patient assessment.
  • Provide physical therapy, including selection and use of assistive equipment.
  • Complete appropriate goal re-assessment and discharge planning.
  • Train and educate patients and family members or other support and documents understanding of treatment plan and goals.
  • Collaborate with patient and other healthcare personnel or outside agencies regarding coordination of care.
  • Document and submit patient reports and charges within the established timeframe.
  • Supervise Physical Therapist Assistants and Rehab Techs.
