Requirements

  • Graduate of an accredited curriculum with a minimum of a Bachelor of Science Degree in Physical Therapy.
  • Must be licensed by the Connecticut Board of Physical Therapy Examiners.
  • New graduate will be considered.

Responsibilities

  • Evaluation – Obtains pertinent medical and social history.
  • Performs thorough and accurate evaluations using techniques to objectively measure physical and functional status.
  • Determines functional and measurable short and long term goals and designs an appropriate treatment program.
  • Considers discharge options at time of initial evaluation.
  • Treatment – Performs treatment as outlined in the initial evaluation.
  • Modifies treatment as necessary, updates goals and re-evaluates discharge plans throughout the course of treatment.
  • Demonstrates thorough knowledge of treatment techniques and applies appropriately.
  • Treatment should include patient and family education and communication/interaction with appropriate team members.
  • Documentation – Clinical documentation is timely, objective and complete.
  • Administrative documentation including but not limited to billing and outcome measures is accurate and timely.
  • Education – Assumes personal responsibility for continuing education and professional development.
  • Provides education to therapy staff, system employees and community.
  • Supervision – Supervises the patient care activities of assistants, aides and volunteers.
  • Delegates routine care to supportive personnel as appropriate.
  • May supervise students.
