About The Position
Episcopal Homes of Minnesota has been a trusted leader in senior care for over 130 years. Nestled in the heart of St. Paul’s Midway neighborhood, our vibrant campus offers a full continuum of care—including Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Home Care services. We are currently seeking a Physical Therapist to join our dedicated Therapy team. In this role, you’ll deliver personalized therapy services that enhance mobility, function, and overall quality of life for older adults in a supportive, community-centered environment. Requirements
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license in the state of Minnesota
- New Grads Welcomed!
- Strong problem-solving skills, empathy, and clinical reasoning abilities
- Familiarity with electronic medical record systems
Nice To Haves
- Senior living or geriatric care experience preferred not required
Responsibilities
- Conduct evaluations and create individualized care plans
- Deliver therapeutic interventions to improve strength, balance, and mobility
- Educate residents, caregivers, and families on safety and home exercise programs
- Track progress and adjust plans based on functional outcomes
- Collaborate with interdisciplinary care teams
- Supervise Physical Therapist Assistants
- Document care accurately in accordance with EMR and regulatory standards
