Requirements

Graduation from an Accredited Physical Therapy Program (DPT) required.
PT License in PA.
Ability to meet physical demands of the position (lifting, transferring residents, prolonged standing/walking).
Familiarity with early intervention practices and patient care techniques.
Excellent communication skills, both verbal and written.
Ability to work collaboratively within a multidisciplinary team environment.

Nice To Haves

Previous senior living experience working with senior populations is highly desirable.

Responsibilities

Evaluates and conducts medically prescribed physical therapy treatment programs.
Provides educational information to residents in regard to injury prevention, ergonomics, restorative therapy and rehabilitation therapy.
Supervises physical therapy assistants and aides in performing treatment.
Performs initial and ongoing assessments of resident's condition.
Establishes a plan of care which is appropriate to problems identified and involves the resident according to his or her capabilities and desires.
Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in residents.
Performs therapy interventions utilizing standard physical therapy techniques and skills as appropriate for the age and condition of the resident.
Directs and supervises physical therapy assistants, students, and other support personnel.
Assesses competence, delegates specific tasks, and establishes channels of communication.
Identifies and documents goals, anticipated progress and plans for reevaluation.
Establish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function.
Maintain communication among the treatment team, physician, patient, and family members as applicable.
Maintain thorough and timely documentation of services provided to ensure proper billing.
