Midtown Oaks Post Acute is hiring a Full-Time Physical Therapist (PT) to join our skilled nursing and post-acute rehabilitation team in Sacramento. Requirements

  • Active California Physical Therapist (PT) license (or pending)
  • Skilled nursing or post-acute experience preferred, but not required
  • Strong communication, documentation, and time-management skills
  • Team-oriented, compassionate, and patient-focused approach

Responsibilities

  • Evaluate and treat patients in a skilled nursing / post-acute rehabilitation setting
  • Develop and implement individualized treatment plans
  • Complete evaluations, progress notes, and discharge documentation
  • Collaborate with nursing, therapy, and interdisciplinary care teams
  • Educate patients and families on therapy goals and home exercise programs
