Midtown Oaks Post Acute is hiring a Full-Time Physical Therapist (PT) to join our skilled nursing and post-acute rehabilitation team in Sacramento. Requirements
- Active California Physical Therapist (PT) license (or pending)
- Skilled nursing or post-acute experience preferred, but not required
- Strong communication, documentation, and time-management skills
- Team-oriented, compassionate, and patient-focused approach

Responsibilities
- Evaluate and treat patients in a skilled nursing / post-acute rehabilitation setting
- Develop and implement individualized treatment plans
- Complete evaluations, progress notes, and discharge documentation
- Collaborate with nursing, therapy, and interdisciplinary care teams
- Educate patients and families on therapy goals and home exercise programs
