About The Position

A qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients in their homes, following physician’s orders. This role involves evaluating patients using diagnostic muscle, nerve, joint, and functional ability tests, preparing initial assessments and periodic re-evaluations, and developing written problem lists and plans of care. The therapist will recommend modifications to the plan of care to the physician as needed, document clinical notes, and treat patients to address impairments and functional limitations with the goal of achieving maximum functional independence. Key aspects include instructing and recommending patients and caregivers in the use of prosthetic, orthotic, and durable medical equipment, and educating them on the treatment plan and goals. The position also requires participation in physical therapy staff meetings, in-service presentations, case conferences, and the development and revision of physician’s Plans of Treatment, while ensuring appropriate safety practices and equipment maintenance.

Requirements

  • Graduate of an accredited Physical Therapy program.
  • Current, valid Physical Therapy license issued by NYS Department of Education.
  • One year of practical experience as a licensed physical therapist.

Responsibilities

  • Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
  • Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
  • Recommend modification of plan of care to physician, as needed.
  • Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
  • Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
  • Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).
  • Include and educate the patient and caregiver(s) regarding treatment plan and goals.
  • Attend physical therapy staff meetings and in-service presentations.
  • Participate in case conferences.
  • Participate in the development and periodic revision of the physician’s Plan of Treatment.
  • Assure appropriate safety practices and maintenance of equipment.
  • Other duties as assigned.
