About The Position

Physical Therapist Whitney Rehabilitation Care Center is looking for a physical therapist to join their wonderful team! Whitney Rehabilitation Care Center has been providing excellent care to the community for over 50 years. Privately owned and operated, we offer short and long term skilled nursing care and rehabilitation as well as outpatient therapy. Whitney Rehabilitation Care Center has been given a 5 Star rating for Quality Measures by the Center for Medicare and Medicaid Services. We are charter members of the Yale Preferred Provider Post-Acute Care Network and recognized as a Tier 1 provider for Hartford Healthcare. Summary of the Registered Physical Therapists (RPT): The Physical Therapist will carry out an individualized program of physical therapy including assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.

Requirements

• Current Connecticut License as a PT (Physical Therapist)
• Current CPR
• At least one (1) experience working in a Skilled Nursing Facility preferred.

Responsibilities

• Creates, implements, and executes patient treatment plans.
• Communicates with patients, families, referring physicians, and other members of the team.
• Provides timely written documentation per facility and department requirements and as required by insurance provider to meet licensing, certification, regulatory, and reimbursement requirements.
• Develops and enhances clinical and professional skills through knowledge and professional associations.
• Supports and participates in department operations and development.
• Reports any changes in patient status or needs to Director of Rehab and interdisciplinary team as appropriate.
• Supervises Physical Therapy Assistants with the goal of ensuring optimal patient care and achievement of outcomes.
