Requirements

  • Active Wisconsin Physical Therapist license
  • Strong clinical, communication, and documentation skills
  • A passion for working with older adults in assisted living
  • Dependability, adaptability, and a team-oriented mindset
  • Graduate of an accredited school with a minimum of a bachelor’s degree in Physical Therapy.
  • Certification or license as required by the State of Wisconsin or applicable state to practice as a Physical Therapist.

Responsibilities

  • Evaluating residents and creating individualized treatment plans
  • Supporting residents in improving mobility, balance, strength, and functional independence
  • Collaborating with interdisciplinary teams for resident-centered care
  • Documenting progress according to regulatory and company standards
