About The Position

Assess, plan, organize, and participate in rehabilitative programs that improve mobility, relieve pain, increase strength, and improve or correct disabling conditions resulting from disease or injury. As a Physical Therapist, you will perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention. You will plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients. You will evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. Additionally, you will apply concepts of infection control/universal precautions in performing patient care, assure continuity of quality patient care delivered with appropriate documentation, provide effective communication to patient/family, team members and other health care professionals, promote agency philosophy and administrative policies, and maintain and complete all documentation within company stated guidelines.

Requirements

  • Successful completion of accredited physical therapy program and state licensure exam.
  • Valid Physical Therapy license.
  • 1 year of home health experience preferred.
  • CPR certification.
  • Valid Driver’s license.
  • Valid Auto Insurance.
  • HomeCare HomeBase experience preferred.
