In this role, you will help ensure Correctional Health Services (CHS) can fulfill its mission of providing medically necessary, evidence-based integrated health care, including physical therapy, to patients going through the judicial system. It ensures CHS provides appropriate medical services that may emerge during a patient's stay in the county jail system, thus preventing injury, impairment, functional limitation, and disability. You will also help promote and maintain health, wellness, fitness, and quality of life in all populations. Correctional Health Services is an industry-leading healthcare organization for the incarcerated population within Maricopa County jails, providing 24/7 medical, dental, mental health, psychiatric care, and substance abuse treatment, i.e., Medication Assisted Treatment (MAT), and Opioid Treatment Program (OTP). As a Physical Therapist, you will initiate physical therapy services upon receipt of a provider's order, examine patients with impairment, functional limitation, disability, or other health-related conditions to determine a diagnosis, prognosis, and intervention. You will review physician's referrals and patient medical records to help determine diagnosis and orders for physical therapy treatment, perform and document initial exams, and evaluate data to clarify and substantiate patient complaints, problems, and functional limitations. You will also plan, prepare, and carry out individually designed plans of care (POC) to improve, restore or maintain physical functioning, alleviate pain, or prevent physical dysfunction in patients. Your role will involve instructing patients on a POC to be continued on their own and informing them that this is to be the primary means of resolving their functional limitations. You will communicate with referring providers regarding patient compliance with POC and alleviate impairment and functional limitation by designing, implementing, and modifying therapeutic interventions. Additionally, you will work with, help train, and supervise care for one or more PT Techs.
