About The Position

The Physical Therapist at Marysville Post Acute is responsible for evaluating, planning, and administering physical therapy treatments to assist patients in their recovery process. This role involves working closely with a multidisciplinary team to create individualized treatment plans that promote mobility, strength, and overall well-being for residents in a skilled nursing facility.

Requirements

  • Licensed Physical Therapist in the state of practice.
  • Experience in a skilled nursing or rehabilitation setting preferred.
  • Strong knowledge of physical therapy principles and practices.
  • Excellent communication and interpersonal skills to work effectively with patients and team members.
  • Ability to develop and implement individualized treatment plans.

Nice To Haves

  • Experience with geriatric patients.
  • Familiarity with electronic health record (EHR) systems.
  • Certification in specialized therapy techniques (e.g., manual therapy, aquatic therapy).

Responsibilities

  • Conduct comprehensive physical therapy evaluations to assess patients' physical condition and functional abilities.
  • Develop individualized treatment plans based on assessment findings, patient goals, and best practices.
  • Administer physical therapy treatments, including exercises, manual therapy, modalities, and other interventions to improve strength, flexibility, balance, and coordination.
  • Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes.
  • Educate patients and their families on treatment goals, therapeutic exercises, and strategies for continued progress after discharge.
  • Collaborate with other healthcare professionals, including physicians, nurses, occupational therapists, and speech therapists, to ensure a cohesive approach to patient care.
  • Maintain accurate and timely documentation of patient care, in compliance with facility, state, and federal regulations.
  • Participate in team meetings, case conferences, and in-service training to enhance professional development and stay current with best practices in physical therapy.
  • Ensure a safe and supportive environment for all residents, adhering to facility policies and procedures.
