About the position

This position is responsible for therapy evaluations, treatment planning, implementation and discharge planning for clients in the home setting in accordance with established facility/agency policies while maintaining the highest degree of quality client care. As a member of the multi-disciplinary team, the therapist assumes responsibility for communication of client related issues for appropriate team members or other facility/agency staff and participates in the collection and evaluation of data necessary to assess the appropriateness of care at home. The Physical Therapist also assumes responsibility for those clients being treated by home health aides and may be assigned to orient staff and students. The Physical Therapist works with the clients and family in the home setting without direct supervision.

Responsibilities

  • Perform comprehensive assessments to determine specific therapy needs.
  • Develop plans of care and coordinate care with other disciplines.
  • Provide direct patient care in the implementation of outcome oriented physical therapy interventions under the physician ordered Plan of Care.
  • Complete, accurate, and timely documentation.
  • Promote and practice patient/family, personal and environmental safety.
  • Participate in relevant educational activities to provide competent care.
  • Display professionalism in the work environment.
  • Maintain productivity as per agency standard.
  • Comply with all applicable Federal, State, and local laws, regulations, and CHAP requirements.
  • Maintain confidentiality of all client demographics, medical and financial information.
  • Ensure proper handling and disposal of confidential documents and adhere to all HIPAA rules and regulations.
  • Adhere to all policies and procedures including regulatory requirements.

Requirements

  • Bachelor's Degree in Physical Therapy from an institution approved by the state board.
  • 1 year Therapy with adult physical disabilities (preferred).
  • Ability to add, subtract, multiply, and divide in all units of measure.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Strong written and verbal communications skills.
  • Ability to understand, read, write, and speak English.
  • Demonstrated knowledge and skills necessary to provide appropriate care to clients ranging in age from adolescents to geriatrics.
