About The Position

Discovery Rehabilitation and Living in Salmon, ID, is a skilled nursing facility with long term care, post-acute rehab, and geriatric outpatient therapy services provided. Requirements

• Licensed Physical Therapist.
• Open to PTs at all experience levels.

Responsibilities

• Provide physical therapy services to patients in a skilled nursing facility.
• Collaborate with a multidisciplinary team to develop and implement patient care plans.
• Conduct assessments and evaluations of patients' physical abilities and needs.
• Document patient progress and adjust treatment plans as necessary.
• Educate patients and their families on treatment plans and exercises.
