About The Position

BAYADA Home Health Care is seeking a dedicated Physical Therapist (PT) to join the Sierra Vista Visits team. In this role, you will work with clients in their homes, developing and implementing personalized treatment plans to help them achieve their rehabilitation goals. As a vital member of a multi-disciplinary healthcare team, you will provide skilled nursing and rehabilitative care, ensuring clients receive the highest quality of home health care with compassion and reliability.

Requirements

• Current Physical Therapist license in Arizona.
• Minimum of one year of recent work experience as a Physical Therapist.
• Graduation from an accredited Physical Therapy program.
• Solid computer skills; experience with electronic medical records preferred.

Nice To Haves

• Experience in home health care settings.
• Additional certifications in specialized therapy techniques.

Responsibilities

• Perform diagnostic tests and measurements related to mobility, strength, endurance, and safety assessments.
• Develop and implement individualized care plans, including therapeutic exercises and gait training.
• Implement protective interventions to prevent wound formation and promote healing using evidence-based practices.
• Continually assess and revise the Physical Therapy care plan as needed.
• Educate clients and their families on rehabilitative care and activities to promote health and independent living.
• Document observations, interventions, and evaluations accurately using electronic medical records.
