About The Position

The Physical Therapist will provide high-quality rehabilitation services to residents, focusing on restoring mobility, improving strength and function, and enhancing overall quality of life. This role involves comprehensive assessments, personalized treatment plan development, and implementation of therapeutic interventions. The therapist will conduct one-on-one sessions, monitor progress, and collaborate with a multidisciplinary team to coordinate care. Additionally, the role includes educating residents and families on exercise programs and safety, documenting services, and participating in care conferences and training sessions. The Physical Therapist is expected to contribute to a positive work environment through professionalism, teamwork, and effective communication.

Requirements

  • Current state licensure or eligibility for licensure as a Physical Therapist in the state of New Mexico.
  • Strong clinical reasoning and decision-making skills, with proficiency in evaluating and treating a diverse range of conditions and diagnoses.
  • Ability to build rapport with residents through a compassionate and caring demeanor.
  • Excellent interpersonal and communication skills, with the ability to collaborate effectively with residents, families, and team members.
  • Commitment to providing high-quality, resident-centered care with empathy and compassion.

Nice To Haves

  • 0-3 years of clinical experience in a long term care, skilled nursing facility, or similar healthcare setting is preferred.

Responsibilities

  • Evaluate Residents' physical abilities, functional status, and rehabilitation needs through comprehensive assessments.
  • Develop personalized treatment plans based on assessment findings, resident goals, and evidence-based practice guidelines.
  • Implement therapeutic interventions to address impairments, maximize functional independence, and promote safe mobility.
  • Provide one-on-one physical therapy sessions to residents, utilizing a variety of therapeutic techniques and modalities.
  • Monitor resident progress and adjust treatment plans as needed to optimize outcomes and achieve rehabilitation goals.
  • Collaborate with team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate care and ensure continuity of services.
  • Educate residents and their families on exercise programs, safety techniques, and home exercise regimens to support ongoing rehabilitation and prevent re-injury.
  • Document all physical therapy services provided, including initial evaluations, progress notes, and discharge summaries, in accordance with facility policies and regulatory requirements.
  • Participate in resident care conferences, team meetings, and in-service training sessions to share expertise, contribute to care planning, and stay informed about best practices in rehabilitation.
  • Contribute to a positive and supportive work environment by demonstrating professionalism, teamwork, and effective communication skills.
  • Perform all other duties assigned.
