  • Valid Physical Therapist License

Responsibilities

  • Treatment Planning: Develop effective treatment plans and obtain approval for services from referring physicians.
  • Patient Care: Treat patients according to physician treatment plans.
  • Training Support: Assist nursing department with training Restorative Aides and supervise Physical Therapy Assistants in patient care activities.
  • Communication: Maintain clear communication with supervisors and health team members regarding patient progress, problems, and plans.
  • Meetings Participation: Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
  • Documentation: Record evaluations, daily treatment notes, weekly progress notes, and recertification documentation as per PT Board requirements.
  • Adjustments: Record treatment changes according to policies and procedures.
  • Family Instruction: Instruct patients’ families or nursing staff in maintenance programs and caregiver training for discharge preparations.
  • Discharge Planning: Participate in discharge planning processes.
  • Equipment Recommendations: Recommend and facilitate the ordering of necessary durable medical equipment to assist patient mobility.
  • Equipment Maintenance: Report any issues with department equipment to ensure proper functioning.
