Residential Home Health and Hospice is seeking a Physical Therapist (PT) to provide skilled physical therapy and rehabilitation therapy to patients in the comfort of their homes in the Lock Haven, PA area. Requirements
- Licensed Physical Therapist in Pennsylvania
- Minimum 1 year of clinical experience
- Valid driver’s license and reliable transportation
Responsibilities
- Deliver skilled physical therapy and rehabilitation therapy in patients’ homes
- Develop and implement individualized treatment plans
- Collaborate with an interdisciplinary care team
- Document patient care using EMR system (HCHB)
