Requirements

• Bachelor’s, Master’s, or Doctorate in Physical Therapy from an APTA-accredited program
• Active Physical Therapist (PT) license in the state of practice or PT Licensure Compact eligibility
• Home health or rehab experience (1+ year required)
• Current CPR certification
• Strong communication, clinical judgment, and time-management skills
• Knowledge of OASIS and home health regulations preferred
• Valid driver’s license, reliable transportation, and current auto insurance

Responsibilities

• Perform home health physical therapy evaluations, including OASIS assessments
• Develop and update individualized Plans of Care (POC)
• Provide skilled home health physical therapy for orthopedic, neurological, and neuromuscular conditions
• Deliver interventions focused on mobility, balance, strength, gait training, and fall prevention
• Educate patients and caregivers on safe transfers, home exercise programs, and use of durable medical equipment
• Complete accurate and timely documentation in the EMR
• Communicate with physicians and interdisciplinary care teams
• Supervise PTAs and home health aides, as applicable
• Follow all home health regulations, safety standards, and infection control policies
