About The Position

We’re seeking dedicated Physical Therapists (PTs) to join our growing team for part-time or per diem visits. Requirements

• Licensed Physical Therapist (PT)
• Current CPR certification
• Physical exam (within last 12 months)
• TB test (within last 12 months)
• Valid driver’s license
• Must attend orientation at our Taunton, MA office

Nice To Haves

• Minimum 1 year of PT experience preferred
• OASIS and WellSky (Kinser) experience preferred (training provided)

Responsibilities

• Evaluate patients’ physical conditions, limitations, and rehabilitation needs
• Develop and implement individualized treatment plans
• Provide therapy in accordance with the physician’s Plan of Care
• Supervise and coordinate care with PTAs and HHAs
• Maintain accurate documentation and progress notes
• Educate patients and caregivers on exercises and equipment use
• Assess home environments and recommend modifications
• Collaborate with nurses, social workers, and other healthcare providers
• Submit visit notes on time while maintaining HIPAA compliance
