Assisted Living Physical Therapist Responsibilities As an Assisted Living PT, you will: Perform comprehensive physical therapy evaluations in assisted living communities Develop and implement individualized Plans of Care (POC) Provide skilled geriatric physical therapy and assisted living rehab services Treat orthopedic, neurological, balance, and mobility impairments Deliver interventions focused on: Fall prevention Gait training Strength and balance retraining Functional mobility and ADL performance Educate residents and caregivers on: Safe transfers Assistive devices Home exercise programs Document accurately and efficiently in EMR Collaborate with physicians, nursing staff, and interdisciplinary rehab teams Supervise PTAs when applicable Ensure compliance with assisted living and senior care regulations This PRN Physical Therapist position is ideal for clinicians who enjoy working in assisted living rehab, senior housing communities, and geriatric care settings. Requirements
- Bachelor’s, Master’s, or Doctorate in Physical Therapy from an APTA-accredited program
- Active Texas Physical Therapist license (or Compact eligibility)
- CPR certification
- Strong clinical judgment and documentation skills
Nice To Haves
- 1+ year experience in Assisted Living, SNF, Home Health, or Geriatric Rehab preferred
- Experience with senior care and assisted living regulations preferred
