About The Position
Inabia is seeking a compassionate and motivated Physical Therapist to provide rehabilitation therapy services to residents within a senior living and skilled nursing community. The therapist will help residents maintain or regain mobility, independence, and overall physical health.
Requirements
- Active Physical Therapist License
- Degree from accredited Physical Therapy program
- Experience in rehabilitation or senior living environment preferred
Responsibilities
- Conduct comprehensive physical therapy evaluations to determine patient mobility limitations and rehabilitation needs.
- Develop individualized treatment plans to improve strength, flexibility, balance, and coordination .
- Provide therapeutic interventions including mobility training, strength exercises, manual therapy, and balance rehabilitation .
- Assist residents recovering from surgery, illness, stroke, or chronic conditions .
- Monitor therapy progress and update treatment programs based on patient performance and goals.
- Educate residents and caregivers on safe mobility practices, fall prevention, and home exercise programs .
- Maintain accurate therapy documentation including evaluations, treatment notes, and discharge summaries .
- Work collaboratively with interdisciplinary care teams including nursing staff, physicians, occupational therapists, and speech therapists .
- Participate in care planning meetings and support safe discharge planning.
