About The Position
Sunnyvale Post Acute is a compassionate, resident‑centered skilled nursing and rehabilitation community committed to delivering exceptional care. Our therapy team is dedicated to supporting patients in regaining their highest level of independence, mobility, and quality of life. We are seeking a talented PRN Weekend Physical Therapist to join our rehab department. The PRN Physical Therapist evaluates, plans, and provides patient‑centered physical therapy services for individuals requiring rehabilitation in a post‑acute/long-term care setting. You will collaborate closely with an interdisciplinary team to ensure the best rehabilitation outcomes for our residents.
Requirements
- Current California Physical Therapist (PT) license
- Graduate of an accredited PT program (DPT or MSPT)
- Strong clinical reasoning and communication skills
- Ability to work independently and collaboratively
Nice To Haves
- Previous SNF or post‑acute rehab experience preferred (not required)
Responsibilities
- Complete PT evaluations and develop individualized treatment plans
- Provide effective, evidence‑based therapeutic interventions
- Document treatments, progress notes, and evaluations in accordance with facility and regulatory requirements
- Communicate patient progress with interdisciplinary team members
- Educate patients, families, and caregivers on therapy goals, safety, and home programs
- Ensure safe, ethical, and high‑quality care at all times
