You’ll provide skilled, compassionate therapy services to home health patients, teach family members proper care techniques, and collaborate closely with the care team and physicians.
Requirements
- Active Physical Therapist (PT) license
- Minimum 1 year of PT experience (Home Health/OASIS preferred)
- Physical (within 6 months of first patient contact)
- Current TB test (within last 12 months)
- Current CPR certification
- Valid driver’s license & reliable transportation
- Must be available to attend orientation at our Willowbrook office
- Available Monday–Friday, 8 AM–5 PM

- WellSky (Kinnser) EMR experience a plus
Responsibilities
- Evaluate patients within 72 hours of receiving a physician referral
- Develop and implement individualized Plans of Care
- Provide therapy services and supervise PTAs and HHAs
- Communicate effectively with physicians and the interdisciplinary team
- Maintain accurate, timely documentation in EMR
- Evaluate treatment effectiveness and update care plans as needed
- Participate in interdisciplinary team meetings and continuing education
- Maintain patient confidentiality per HIPAA standards
