Requirements
- Licensed Physical Therapist (PT) in Virginia (or eligible)
- Doctor of Physical Therapy (DPT) required
- Minimum of 1–2 years of clinical experience required
- Strong communication, organization, and time management skills
- Ability to manage a consistent caseload independently
Nice To Haves
- Home health experience preferred, not required
Responsibilities
- Provide home health physical therapy services to adult and geriatric patients
- Perform evaluations and develop individualized care plans
- Deliver rehabilitation services focused on strength, balance, and mobility
- Educate patients and caregivers on treatment plans
- Collaborate with physicians and interdisciplinary care teams
- Complete accurate and timely EMR documentation
