Requirements

  • Graduate of an APTA-approved Physical Therapy program
  • Active, unrestricted Physical Therapist license in the state of practice
  • At least one year of experience in a healthcare setting
  • Willingness to travel within the assigned coverage area (up to 50%)
  • Reliable transportation, valid driver’s license, and current auto insurance
  • Ability to lift 50–100 lbs and perform essential physical functions of the role

Responsibilities

  • Deliver hands-on, patient-centered care in the comfort of patients’ homes
  • Evaluate functional needs and create personalized plans of care
  • Track progress, adjust treatment plans, and celebrate patient wins
  • Collaborate with physicians, caregivers, and interdisciplinary teams
  • Mentor and guide therapy assistants and support staff
  • Complete timely, accurate documentation to support excellent outcomes
  • Act as a clinical ambassador for physical therapy in the community
