About The Position

Humboldt Senior Resource Center is looking to hire a Physical Therapist to support our participants in our Redwood Coast PACE program. This position works Monday through Friday, between 8 am and 5 pm. We prefer full time but will consider a part-time schedule. JOB SUMMARY: The Physical Therapist is responsible for providing physical therapy services to Redwood Coast PACE participants, adhering to departmental, professional, and regulatory standards of care. This role does not involve supervision responsibilities and operates under the guidance of the Therapy Services Manager.

Requirements

  • Bachelor’s Degree in physical therapy from an institution approved the Centers for Medicare and Medicaid Services
  • At least one-year of experience working with a frail or elderly population required.
  • Must be licensed by the Physical Therapy Board of California.
  • Valid California Driver’s License and personal auto liability insurance required. Must have a personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above.
  • Current CPR and First Aid certification within six (6) months of hire.
  • Requires clearance of a DOJ and FBI criminal history background check
  • Pre-employment physical and PPD skin test.
  • Requires Motor Vehicle Report verification.

  • Physical abilities sufficient to move between different staff work areas, communicate with co-workers and the public, operate a computer, produce reports, talk on the telephone, and travel to other agency worksites.
  • Ability to spend sufficient work time on feet, lifting and moving participants while using proper body mechanics and lifting techniques when assisting participants.

Responsibilities

  • Perform initial assessments and quarterly reassessments of assigned participants to evaluate functional ability, identify major problems, and establish realistic treatment goals.
  • Develop and implement a therapeutic Plan of Care with ongoing recommendations, coordinating with the Interdisciplinary Team.
  • Deliver appropriate treatment programs and monitor treatment progress for assigned participants.
  • Demonstrate proficiency in the application of treatment modalities, including exercise, and understand indications and contraindications.
  • Analyze participant needs for equipment, orthotics, or prosthetics and provide assistance in procurement as necessary.
  • Participate in a physical therapy maintenance program and collaborate with Therapy staff in implementing it.
  • Participate on the Interdisciplinary Team, provide education to team members, and actively contribute to team meetings.
  • Ensure safe practices, emergency preparedness, and practice good body mechanics.
  • Provide education to participants, family members, or caregivers as ordered by the Primary Care Provider and in accordance with physical therapy standards.
  • Collaborate with external physical therapists for the treatment and education of hospitalized or institutionalized participants.
  • Complete documentation in the medical record accurately and in a timely manner.
  • Contribute to physical therapy program development and implementation based on Redwood Coast PACE needs.
  • Maintain technical and clinical competency according to departmental, organizational, and professional standards.
  • Understand fiscal issues and support meeting budget targets.
  • Comply with safety policies and procedures, reporting any potential or actual unsafe acts or conditions to the Center Manager.
  • Continually seek better ways for delivering services and communicating with participants.
  • Meet or exceed Redwood Coast PACE customer service targets.
  • Demonstrate respect for and promote participant rights, including dignity, self-determination, access to care, confidentiality, and independence.
  • Collaborate effectively with staff, peers, and contractors to meet Redwood Coast PACE goals and ensure success.
  • Demonstrate proficiency in delivering age-specific care.
  • Conduct staff training and in-services as needed.
  • Acts in a manner consistent with agency values.
