About The Position

ATC Healthcare is seeking an experienced Home Health Physical Therapist (PT) for a travel assignment in Novato, California. This role provides in-home physical therapy services to adult and geriatric patients, focusing on functional mobility, rehabilitation, safety, and independence in the home setting. The PT will deliver comprehensive home-based care, develop individualized treatment plans, perform functional assessments, and collaborate with interdisciplinary teams to support recovery and long-term patient outcomes. This position requires strong autonomy, critical thinking, and the ability to manage complex caseloads independently in a community-based care environment.

Requirements

Graduate of an accredited physical therapy program.
Active California Physical Therapist license (required at submission).
BLS certification (AHA or American Red Cross).
Valid Driver’s License (Class C) and active automobile insurance required.
Minimum 2 years of experience required (5 years recent relevant experience strongly preferred).
Travel experience required.
Strong independent clinical judgment and problem-solving skills.
Excellent communication, organization, and time management abilities.
Proficient with required technology and documentation systems.

Nice To Haves

Home Health experience preferred.

Responsibilities

Perform in-home physical therapy evaluations and reassessments.
Develop and implement individualized treatment plans.
Deliver skilled therapy focused on mobility, strength, balance, and function.
Conduct safety and environmental assessments in patient homes.
Educate patients and families on exercises, safety, and care plans.
Complete OASIS documentation and care coordination.
Manage discharge planning and long-term goals.
Collaborate with interdisciplinary care teams (PT, social services, home health staff).
Maintain accurate and timely clinical documentation.
