Your Impact & Responsibilities: Rapid Evaluation: Conduct comprehensive patient evaluations within 24 hours (or 1 business day) of physician referral to jumpstart the recovery process. Evidence-Based Treatment: Develop and implement individualized plans focused on strength, balance, pain management, and functional mobility. Clinical Leadership: Supervise Physical Therapy Assistants (PTAs) in direct patient care, ensuring strict adherence to the California State Practice Act. Interdisciplinary Synergy: Participate in Patient Care Conferences and Utilization Review (UR) meetings to ensure seamless transitions and integrated care. Equipment Advocacy: Recommend and facilitate the ordering of Durable Medical Equipment (DME) to enhance resident safety and independence. Empowerment: Educate patients, families, and nursing staff on maintenance programs and caregiver training in preparation for a successful discharge. Precision Documentation: Maintain accurate, timely evaluations and progress notes in accordance with state and payer requirements. Qualifications: Licensure: Current and in good standing California Physical Therapist (PT) license.
