Requirements

  • Licensed Physical Therapist (PT) in Maryland or eligible
  • Doctor of Physical Therapy (DPT) required
  • New grads welcome – mentorship provided
  • Interest in home health or community-based care
  • Strong communication and time management skills
  • Ability to work independently while staying connected to a team

Responsibilities

  • Provide home health physical therapy services to adult and geriatric patients
  • Perform evaluations and develop individualized care plans
  • Deliver rehabilitation services focused on strength, balance, and mobility
  • Educate patients and caregivers on treatment plans
  • Collaborate with physicians and interdisciplinary care teams
  • Complete accurate and timely EMR documentation
