About The Position

Theracare is currently seeking a motivated Physical Therapist to deliver exemplary care and enthusiasm in working with our adult population in both the Skilled Nursing and Home Health Care settings. This is a Part-time/PRN position for a skilled nursing facility in Portage, IN with home health care.

Requirements

• Bachelor of Science or advanced degree in Physical Therapy
• Current state license/certification
• Current registration with the American Physical Therapy Certification Board
• Excellent oral, written communication and interpersonal skills
• Willingness to provide therapy services in locations including, but not limited to, Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities or Home Health Care
• Demonstrate competency in applying the principles, methods, materials and equipment used in specific therapy discipline
• Knowledge of accreditation standards and compliance requirements

Nice To Haves

• Prefer minimum one (1) year experience in rehabilitation
• New graduates are welcome to apply

Responsibilities

• evaluate and treat patients, in accordance with the orders from the physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination, body alignment, to relieve pain, improve movement and function
• work as a member of a team providing support and information within the area of Physical Therapy
• provide therapy services in locations including, but not limited to, Skilled Nursing Facilities, Outpatient Facilities, Long Term Care Facilities or Home Health Care
