Home Health Physical Therapist (PT) Opportunities in Pueblo, CO – Full-Time Available! Compensation: $125,000 annual salary with $15,000 bonus Bonus Structure: Relocation Bonus: Up to $15,000 for candidates who reside outside of Pueblo, Colorado Sign-On Bonus (Local Candidates): $15,000 total , paid as follows: $2,500 after 90 days of employment $2,500 after 6 months $10,000 after 12 months Schedule: Full-time Monday through Friday Location: Pueblo, Colorado At Interim HealthCare® , we’re redefining what personalized care looks like. Requirements

  • Graduate of an accredited Physical Therapist Program and active PT license in Colorado
  • In the last five (5) years, two (2) years of experience as a physical therapist that is relevant to the patient population assigned preferred.
  • CPR Certification (demonstration course required)
  • Knowledge of state and federal home health regulations
  • Good clinical judgment, strong interpersonal skills, excellent time management and compassion

Responsibilities

  • Provide home-based physical therapy to patients who have lost mobility, strength and functionality due to an illness or injury
  • Work as part of a home health team which may include an RN, LPN, CNA, HHA, OT, COTA, PTA and SLP, focused on the patient’s plan of care and goals
  • Observe movements, conduct assessments, document progress and modify care plan as needed
  • Assist patient with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients and family on their plan of care, therapy goals, home exercises, and self-care
