This role offers clinical autonomy while working alongside a collaborative interdisciplinary team. If you enjoy building strong patient relationships, managing your own schedule, and seeing real functional outcomes this is the role for you. Responsibilities Perform comprehensive physical therapy evaluations Develop and implement individualized treatment plans Provide skilled therapy to improve mobility, balance, strength, and safety Educate patients and caregivers on home exercise programs Complete accurate and timely EMR documentation Collaborate with nurses, physicians, and care team members Supervise PTAs as needed Ensure compliance with Medicare, HIPAA, and agency policies Qualifications Graduate of an accredited Physical Therapy program Active DC Physical Therapist license (Required) Minimum 3 years clinical experience (Home Health preferred) OASIS experience strongly preferred EMR experience (WellSky/Kinnser or Homecare Homebase preferred) Strong clinical judgment and communication skills Valid driver’s license & reliable transportation Why You’ll Love It Here If you're a compassionate, driven Physical Therapist looking for flexibility, stability, and meaningful one-on-one patient care we want to hear from you. Requirements
- Graduate of an accredited Physical Therapy program
- Active DC Physical Therapist license (Required)
- Minimum 3 years clinical experience (Home Health preferred)
- Strong clinical judgment and communication skills
- Valid driver’s license & reliable transportation
Nice To Haves
- OASIS experience strongly preferred
- EMR experience (WellSky/Kinnser or Homecare Homebase preferred)
Responsibilities
