About The Position

As a Physical Therapist (PT) with GrandCare, you will provide in-home physical therapy services according to a physician’s plan of care. You’ll help patients regain mobility and independence after surgery, making a real difference in their recovery.

Requirements

• Current California Physical Therapy License
• 2+ years of physical therapy experience (home health preferred)
• CPR certification
• Valid driver’s license and insured vehicle

Responsibilities

• Perform home health physical therapy evaluations and develop care plans
• Provide therapeutic treatment and instruct patients/caregivers
• Monitor progress and report changes to physicians
• Participate in discharge planning and interdisciplinary case conferences
• Supervise Physical Therapy Assistants per state regulations
