About The Position
Physical Therapist (PT) – Long-Term Care LP Therapy LP Therapy is seeking a licensed Physical Therapist (PT) to provide skilled therapy services in a Long-Term Care setting focused on restoring mobility, strength, and function.
Requirements
- Bachelor's degree in Physical Therapy
- current state PT license
- strong knowledge of Medicare guidelines
- effective communication and teamwork skills
Nice To Haves
- LTC experience preferred
Responsibilities
- review physician referrals and medical records
- evaluate strength, mobility, and functional ability
- develop and modify treatment plans
- administer therapeutic exercises and modalities
- educate patients and families
- complete timely Medicare documentation
- collaborate with care teams
- supervise PTAs and aides as required
- support quality outcomes and productivity standards
