About The Position

PHYSICAL THERAPIST CommuniCare Rehab, a member of the CommuniCare Family of Companies has a Full Time opening for a Physical Therapist (PT) at Chardon Healthcare Center, where our therapists are In-House Team Members. This is an exciting opportunity to make a direct impact with our residents through therapy. Requirements

A Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an Accredited Program.
Hold a valid or pending State Physical Therapy License

Responsibilities

Evaluate, Review, and Screen patient’s/resident’s medical charts, evaluations under physician order and develop a treatment plan in accordance with patient’s needs.
Implement treatment plan through direct treatment, as well as education of the plan of care to all team members, including providing necessary supervision for PTAs.
Attend/Lead interdisciplinary meetings to relay clinical findings and make recommendations to other members of rehab team, as well as conducting staff/family training and education when appropriate.
