Physical Therapists (PTs) At Country Meadows, our therapists do more than restore movement—they help residents regain independence, confidence, and quality of life. The Physical Therapist (PT) evaluates, diagnoses, and treats individuals with functional movement, mobility, strength, balance, and physical performance impairments while delivering resident-centered physical therapy services that support rehabilitation and functional independence. The PT works collaboratively with patients, families, caregivers, and the interdisciplinary care team, and provides clinical supervision and oversight of Physical Therapy Assistants (PTAs) as applicable. Services are delivered in accordance with Pennsylvania licensure requirements, professional standards, and organizational policies, with a strong focus on dignity, safety, and individualized care.
