VNA & Hospice of the Southwest Region is looking to hire a per diem Physical Therapist (PT) for our Franklin County service area. The position would be 2 or 3 days, approximately 20 hours, per week. A DAY IN THE LIFE OF A PHYSICAL THERAPIST (PT)
As a PT, you are responsible for evaluating, developing, implementing, and documenting a plan of treatment for patients of all ages. In addition, this position is responsible for serving as a case manager providing supervision of students, physical therapist assistants (PTA), and home care aides.

Requirements

• Bachelor's degree in Physical Therapy from an accredited university or college required.
• 2+ years of PT experience is preferred.
• Valid State Driver's License required.

• Can you get the job done with minimal supervision?

Nice To Haves

• Master's degree preferred.

Responsibilities

• Actively monitors and amends patient visit schedule to maximize productivity and patient needs.
• Performs comprehensive (OASIS and PT) assessments of assigned patients.
• Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
• Plans for and provides skilled physical therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, and assessing the patient's status and progress on each visit.
• As Care Manager and/or team member, coordinates patient care with other clinical team members.
• Supervises and directs clinical activities of PTAs, PT students, and home care aides.
• Completes and coordinates required paperwork.
