About The Position

Physical Therapist (PT) Hugh Chatham Health at Home (formally known as Yadkin Valley Home Health) Area: Forsyth County and surrounding areas Flexible scheduling (4 or 5 days a week, weekend shifts also available) Opportunity for a company vehicle If in personal vehicle, mileage reimbursed at full IRS rate (.725c/mile for 2026) Opportunity for a production based bonus Monthly cellphone stipend Email [email protected] for more information. Education and Formal Training: Successful completion of a PT program approved by the APTA or the Council of Medical Education and Hospitals of American Medical Association or both. If licensed or registered prior to 1966 meets the alternative qualifications cited in the Medicare COP, Section 405.l2 or (1)(2)(3)(4). Licensed in the State of North Carolina as a Physical Therapist. Extended training for new graduates. Knowledge, Skills, and Abilities Required: Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment, and a professional attitude when relating with patients, families, and co-workers at all times. Actively functions as a patient advocate. Must be able to quickly develop thorough knowledge of home health regulations and all relevant policies.

Requirements

Successful completion of a PT program approved by the APTA or the Council of Medical Education and Hospitals of American Medical Association or both.
If licensed or registered prior to 1966 meets the alternative qualifications cited in the Medicare COP, Section 405.l2 or (1)(2)(3)(4).
Licensed in the State of North Carolina as a Physical Therapist.
Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with ill people; uses tact, sensitivity, sound judgment, and a professional attitude when relating with patients, families, and co-workers at all times.
Actively functions as a patient advocate.
