Physical Therapy - Physical Therapist (PT)
About The Position
This position is for a Physical Therapist working in a home health setting with day shifts from 8:00 AM to 4:30 PM. The role involves providing rehabilitation services to patients within a defined geographic territory, primarily supporting individuals in their homes within approximately a 45-mile radius of the Duluth area. The Physical Therapist will evaluate patients, initiate plans of care, and provide treatment aimed at improving mobility, strength, and functional independence. The role focuses heavily on start-of-care assessments and initial evaluations, with some routine follow-up visits. The therapist will also provide clinical oversight and guidance for Physical Therapist Assistants when applicable. The position offers consistent weekly hours with no scheduled weekends or holidays, though occasional on-call coverage may occur when census is low.
Requirements
- Active Physical Therapist licenses in both Minnesota and Wisconsin
- Current Basic Life Support certification
- Bachelor’s Degree in Physical Therapy or a related field
- Strong assessment skills
- Ability to manage patient care independently in a home setting
- Effective communication abilities to coordinate care with interdisciplinary healthcare teams
- Comfortable traveling between patient homes within the assigned service territory
- Professional reliability
- Adaptability
- Ability to work in varying weather conditions
