About The Position

"Navitas Healthcare, LLC" is seeking a dedicated Physical Therapist (PT) to provide patient-centered rehabilitative care. The PT will manage the complete therapeutic process for assigned patients, working closely with physicians and the interdisciplinary care team. Responsibilities include evaluating patient needs, developing individualized treatment plans, and delivering skilled therapy services. The role emphasizes patient engagement, goal achievement, and a positive care experience.

Requirements

• Bachelor’s Degree in Physical Therapy – Required
• Active State License as a Physical Therapist
• Basic Cardiac Life Support (BCLS) certification (must be obtained within 30 days of start date)
• Open to New Graduates – prior experience not required

Responsibilities

• Conduct comprehensive physical therapy evaluations and treatment interventions in accordance with professional standards.
• Assess therapeutic, rehabilitative, and functional status; contribute to individualized plans of care.
• Deliver skilled therapy using evidence-based techniques to support functional improvement.
• Educate patients, families, and caregivers on exercises, therapies, and adaptive/prosthetic devices for continued care.
• Prioritize caseloads, determine daily staffing needs, and coordinate patient assignments and handoffs.
• Collaborate effectively with physicians and the interdisciplinary care team to support optimal patient outcomes.
