Requirements

• Current license as a Physical Therapist in PA
• A heart for patient-centered care
• Reliable transportation
• CPR Certification
• Proficient with computer applications and documentation systems.
• Strong communication skills to interact effectively with patients, families, and team members.
• Ability to manage multiple tasks and adapt to changing patient needs.

Nice To Haves

• At least two years of relevant experience within the last five years.

Responsibilities

• Deliver expert physical therapy on individualized care plans
• Conduct initial and ongoing comprehensive assessments to monitor patient progress.
• Collaborate with interdisciplinary teams to promote optimal outcomes.
• Regularly assess patient status and adjust treatment plans as necessary.
