Requirements

• Active California or Arizona PT license (dependent upon assignment state)
• Prior experience in a Skilled Nursing Facility
• Strong communication & documentation skills

Responsibilities

• Perform patient evaluations and develop individualized treatment plans
• Provide PT services to short-term rehab & long-term care residents
• Improve patient mobility, strength, balance, and functional outcomes
• Document progress and outcomes accurately
• Collaborate with interdisciplinary care teams
