About The Position

The position is for a full-time Licensed Physical Therapist at Advocate Condell Medical Center's Outpatient Rehabilitation department. The role involves interpreting physician referrals and conducting various evaluations including musculoskeletal, neuromuscular, cardiopulmonary, and integumentary assessments. The therapist will establish and implement individualized treatment plans based on evaluation results and functional goals, monitor patient outcomes, and adjust treatment plans as necessary. Responsibilities also include planning for patient discharge, providing education to patients and families, and ensuring all documentation meets department policy and accreditation standards. The therapist will manage a caseload, participate in performance improvement activities, and may supervise physical therapy assistants and students. The position requires physical demands such as lifting, pushing, and transferring patients, and adherence to safety and infection control measures.

Requirements

  • Doctorate degree in Physical Therapy from an accredited school.
  • Licensed Physical Therapist in the State of practice.
  • Basic Life Support (BLS) certification within 90 days.
  • No prior work experience required.
  • Intermediate computer skills.
  • Excellent communication, interpersonal, and organization skills.

Responsibilities
