Shadelands Post Acute is looking for a skilled and compassionate Physical Therapist to join our premier in-house therapy team on a PRN basis. Your Clinical Impact Strategic Evaluations: Develop effective treatment plans and coordinate with referring physicians to secure necessary approvals for services. Hands-On Therapy: Treat residents according to physician-approved plans, focusing on restoring mobility, strength, and independence. Leadership & Supervision: Oversee Physical Therapy Assistants (PTAs) in accordance with the state practice act and assist in training Restorative Aides. Interdisciplinary Collaboration: Participate in patient care conferences and utilization reviews, and communicate progress effectively with the broader clinical team. Comprehensive Documentation: Maintain impeccable records, including evaluations and progress notes, ensuring compliance with PT Board and third-party payer requirements. Discharge Success: Facilitate discharge planning by recommending durable medical equipment (walkers, braces, etc.) and providing essential caregiver training for families. Requirements

  • A valid, current Physical Therapist (PT) License in the state of California.
  • Strong clinical judgment and the ability to work independently.
  • Excellent interpersonal skills to build rapport with residents, families, and colleagues.
