Requirements

  • Graduate of an accredited Physical Therapy program
  • Active PT license in South Carolina
  • 1 year of PT experience (home health experience preferred)
  • New Grads Welcomed
  • CPR certification
  • Valid driver’s license and auto insurance
  • Knowledge of state and federal home health regulations
  • Empathy, clinical judgment, and strong communication skills

Nice To Haves

  • home health experience preferred

Responsibilities

  • Deliver in-home physical therapy to help patients rebuild strength and function
  • Assess movement, document progress, and adapt care plans to meet patient goals
  • Partner with home health teams including RNs, LPNs, CNAs, OTs, and SLPs
  • Provide hands-on therapy, pain management, and tailored exercise plans
  • Educate patients and families on recovery goals and safe techniques
