Requirements
- Graduate of an accredited Physical Therapy program
- Active PT license in South Carolina
- 1 year of PT experience (home health experience preferred) New Grads Welcomed
- CPR certification
- Valid driver’s license and auto insurance
- Knowledge of state and federal home health regulations
- Empathy, clinical judgment, and strong communication skills
Nice To Haves
- home health experience preferred
Responsibilities
- Deliver in-home physical therapy to help patients rebuild strength and function
- Assess movement, document progress, and adapt care plans to meet patient goals
- Partner with home health teams including RNs, LPNs, CNAs, OTs, and SLPs
- Provide hands-on therapy, pain management, and tailored exercise plans
- Educate patients and families on recovery goals and safe techniques
