About The Position

Tice Valley Post Acute is seeking a skilled and compassionate PRN Physical Therapist to join our rehabilitation team. The ideal candidate will provide high-quality, patient-centered care to help residents regain mobility, improve functional independence, and enhance overall quality of life. Requirements

  • Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
  • Current California Physical Therapist license (in good standing)
  • Strong clinical assessment and treatment skills
  • Excellent communication and interpersonal abilities

Nice To Haves

  • Experience in skilled nursing or post-acute care preferred, but not required

Responsibilities

  • Evaluate residents and develop individualized treatment plans based on clinical findings
  • Implement therapeutic interventions to improve strength, mobility, balance, and coordination
  • Monitor patient progress and adjust treatment plans as needed
  • Collaborate with interdisciplinary team members including physicians, nurses, and occupational therapists
  • Educate patients and families on treatment plans, exercises, and safety strategies
  • Maintain accurate and timely documentation in compliance with regulatory standards
  • Ensure adherence to all facility policies and professional guidelines
