About The Position

All Heart Home Care is seeking skilled, motivated Physical Therapists (PTs) and compassionate Physical Therapist Assistants (PTAs) to join our growing home health team. In these roles, you will provide personalized, patient-centered therapy services in patients' homes, helping them improve mobility, reduce pain, restore function, and regain independence following illness, injury, or surgery. Requirements

Active Physical Therapist (PT) license in the state of employment
Minimum of 1–2 years of experience (home health experience preferred)
Active Physical Therapist Assistant (PTA) license in the state of employment
Previous experience in home health or outpatient settings preferred
Strong clinical and assessment skills
Excellent communication and patient education abilities
Ability to work independently and manage time effectively
Compassionate, patient-focused approach
Current CPR certification

Responsibilities

Evaluate patients' physical conditions and develop individualized treatment plans
Implement therapeutic exercises and interventions to improve strength, balance, and mobility
Monitor patient progress and adjust treatment plans as needed
Educate patients and caregivers on exercises, safety, and injury prevention
Document evaluations, treatments, and outcomes accurately
Collaborate with physicians, nurses, PTAs, and other healthcare professionals
Ensure compliance with all healthcare regulations and company policies
Implement treatment plans developed by the supervising Physical Therapist
Assist patients with therapeutic exercises and functional mobility
Monitor patient progress and report changes to the supervising PT
Educate patients and caregivers on exercises, safety, and proper techniques
Document treatments and patient responses accurately and in a timely manner
Maintain a safe, supportive environment during therapy sessions
Comply with all healthcare regulations and company policies
