About The Position
Physical Therapist (PT) – Per Diem | $70 - 85 per Visit 1:1 Homecare Location: Union County, NJ | Flexible Scheduling BrightStar Care of Westfield & Summit is seeking an experienced, compassionate Physical Therapist (PT) to provide one-on-one home health care. Requirements
- Master’s degree (or higher) in Physical Therapy from an accredited program
- Active NJ PT license + CPR/BLS certification
- Minimum 1 year of PT experience (home health preferred)
- Commitment to compassionate, high-quality care

Responsibilities
- Evaluate patients, develop personalized care plans, and provide skilled PT treatments
- Collaborate with physicians & the Director of Nursing
- Teach patients, families, and caregivers safe mobility & exercise techniques
- Monitor progress, document outcomes, and support discharge planning
